Guidelines in several countries allow an SGLT2 (empagliflozin) with a GLP-1 (semaglutide) when type 2 diabetes is not at target, or when heart or kidney benefit is the aim. That is a clinic decision. It is not a cart combo we recommend as a default.

What can add up

Semaglutide slows the gut; empagliflozin increases urine volume. Together, poor intake plus extra losses is how people get dizzy or land in DKA on a sick day. Titrate the GLP-1 as prescribed. Keep fluids. Pause the SGLT2 when you cannot eat.
